We're so pleased that you've decided to visit us at Simply Screening!

For your convenience, we are providing you with essential new patient registration forms. This will enable you to complete the paper work in your home at your leisure and decrease the amount of time you spend in our reception area.

After completing the forms, please fax them to 757 547-7727, or you can mail them back to us at the address listed below. Fax is the preferred method so that we can have ample time before your appointment to review your medical information. If you have extensive old records we should review, print a medical release form, complete and sign it, and return it as well.

You may scan and attach your paperwork to an email sent to drraymond@simplyscreening.com. Please be aware that the email may not keep your material secure.

We will then call you within 24-48 hours of receipt to make your appointment, or you may contact our charming staff at 757 547-2673 (54-SCOPE).

Your First Office Visit*

Plan to arrive at least fifteen minutes prior to your office* appointment time so that your new patient paperwork can be finalized. We believe that the more information that we can have about you and your evaluation to date, the better we may serve you. Please bring the following items to your first appointment:

  • Insurance card(s) and referral forms (if required), as well as co-payment
  • Completed patient forms.
  • Copies of reports from important recent doctors' visits, X-ray films, or lab tests
  • List of all prescription and over-the-counter medications that you are currently taking with dosage and frequency
  • List of medication and environmental allergies

* If you are scheduled for a DAC (direct access colonoscopy), your paperwork will indicate at which facility Dr Raymond will do your colonoscopy: Camelot, Bayside or Virginia Beach General.

Appointment Changes and Cancellations

We realize that your time is valuable, so we will strive to see you right on schedule.

We ask that you provide as much advanced notice as possible when running late for an appointment or when needing to cancel. With advance notice, the time can be scheduled for another patient who needs our care.
